All medical appointments with physicians require a referral from a doctor. **Please ensure your personal email is on the referral form.**

 

1. IUD/Nexplanon patients: All IUD/Nexplanon initial appointments will be virtual consultations unless you are ready with the device to have an insertion or need a removal without consultation. We recommend your doctor use this form:

2. All other patients: Ask a doctor to send a referral to our email or fax, below.

​3. ​We will contact you by email with your appointment date and time. Please ensure your personal email is on the referral form.

4. Appointments with our Allied Health Professionals do not require a referral. ​

• Please note that these appointments are not covered by OHIP, but may be covered by your private health insurance.
